Extend forms for paid workflows

Custom Contact Forms Pro adds payment and pricing utilities to the WordPress form builder for bookings, orders, registrations, donations, waivers, and agreements. Build forms with drag-and-drop fields and live previews, then place them with a Gutenberg block, shortcode, widget, or PHP function.

Take payments and set prices

Accept Stripe payments inside a form using cards, Apple Pay, Google Pay, or Link. A live payment button shows the amount due, and totals are verified on the server. PayPal payments also stay within the form and use server-verified totals.

Set fixed prices, offer product options, or let customers choose an amount. Pricing fields can include quantities and images, with enforced minimums. Percentage or fixed-amount coupons adjust live totals as users make selections. Pro adds no transaction fees; standard Stripe or PayPal processing fees still apply.

Keep receipts and agreement records

For paid orders, attach PDF receipts to notification emails. Pro adds the core builder’s signature to PDF receipts, while consent fields can record versioned agreements. Download any individual submission as a PDF with one click.

A consent or terms field can show scrollable terms, record versioned agreements, and preserve a snapshot of the agreement.

Guide visitors through each form

Multi-step forms can include page breaks, progress indicators, and validation for each step. Conditional logic can show or conceal fields based on answers, including product and pricing fields. Survey and star-rating fields support Likert grids and quick ratings.

Address autocomplete lets visitors select an address as they type and fills the street, city, state, and ZIP fields. It uses your Google API key. SMS confirmations can notify customers when they book or pay through your Twilio account.

Templates, integrations, and setup

Choose from 13 templates for donations, orders, registrations, waivers, agreements, and other workflows, then adjust the fields and settings. Pro also adds connections for MailerLite, Brevo, Kit, ActiveCampaign, and MailPoet alongside the core plugin’s Mailchimp connection.

1. Open the WordPress form builder and choose a template or start a form from scratch.
2. Drag fields into the form, then configure pricing, payments, conditional logic, consent, or other Pro capabilities as needed.
3. Save the form and insert it with a Gutenberg block, shortcode, widget, or PHP function.

The AI form-building option uses the provider connected under Settings > Connectors. It requires WordPress 7.0 or later and stays hidden on earlier versions.
