Contact Form by Supsystic Pro adds conditional logic, advanced fields, A/B testing, and integrations to a drag-and-drop WordPress form builder. Start with an editable template, arrange fields, set labels and required status, and review the live preview as you work.

Design forms and select where they appear

The builder includes more than 25 field types, such as text, email, selection lists, checkboxes, date and time pickers, Google Map fields, and Google reCAPTCHA. The base add-on supports unlimited forms, fields, and submissions. Design controls adjust form width, backgrounds, and colours, with CSS and HTML editors for further control.

Forms adapt to phones, tablets, and desktops. Add one to a post, page, or widget area with its shortcode, or use the Contact Form widget in a sidebar. Shortcodes also work with popup add-ons that support them.

Use Pro fields and conditional paths

Pro conditional logic shows or hides fields based on earlier answers. Additional fields include file upload, rating, address search, WordPress editor, post category, and WooCommerce product categories and attributes such as size or colour. Pro can also register WordPress users through a form or let visitors submit posts.

Google reCAPTCHA is available in the base field set. Pro adds reCAPTCHA v3, hCaptcha, and Cloudflare Turnstile.

Manage submissions and form activity

Configure email notifications with a custom subject, sender, Reply-To address, and message, and send copies to multiple addresses. After submission, display a success message, hide the form, or redirect visitors to a page. A test email option helps check whether your website can send mail.

You can save entries to the plugin's Contacts list and export submissions to CSV. Statistics report views, submits, successful sends, and errors by hour, day, week, or month, with CSV export for the data. Pro A/B testing lets you compare form variants. Mobile notifications can alert you to new leads.

Connect other services

Pro forms can subscribe visitors to services including ConvertKit, ActiveCampaign, GetResponse, Constant Contact, Campaign Monitor, SendGrid, and Salesforce. The plugin also integrates with Popup, Membership, and Google Maps by Supsystic.

Set up a form

In WordPress, install and activate the plugin, then open the Contact Form menu and choose Add New Form. Select a template, add or edit fields, configure the email and success message under Submit Options, and save. Copy the form shortcode into a page or post, or add the Contact Form widget to a sidebar.
