MetForm Pro: WordPress Forms with or without Elementor
MetForm Pro builds forms for tasks ranging from a simple contact request to a longer booking or application. In Elementor, arrange fields as widgets and adjust the layout for mobile, tablet, and desktop. Images or videos can sit below a form, and forms can also be placed with a shortcode in editors that support shortcodes.

Choose fields and arrange a form
Start with a template or a blank layout. The library includes more than 44 templates for contact, booking, calculation, newsletter, event, job application, and multi-step forms. Custom field groups can be saved and reused in other forms, while different input styles help match a site's styling.
Use MetForm Pro for volunteer applications, event registrations, loan applications, restaurant reservations, admission forms, newsletter signups, data erasure requests, product orders, WooCommerce checkouts, subscription forms, patient forms, suggestions, surveys, and post-survey forms.
Guide visitors through questions
For a form with follow-up questions, conditional rules can show or conceal fields based on a visitor's selections or input. Six ready-made conditional form templates offer starting points. Longer forms can be split into multiple steps, and scheduled start and end dates control when a form is available for registrations, surveys, or other time-sensitive tasks.
Quiz and survey forms support interactive questions. Quiz options can assign points and show custom results based on responses. Mandatory consent checkboxes can capture permission before data is stored.
Submissions and reporting
Submissions can be saved in the WordPress administration area, sent to the form administrator, and followed by confirmation emails to users. Field values can also map to post titles, content, and custom post types so visitors can submit articles or reviews from the front end.
The analytics dashboard reports conversion rate, traffic sources, top referrers, geographic breakdown, device and browser statistics, and hourly submission patterns. Analytics data stays inside WordPress without requiring Google Analytics, third-party scripts, or cookies. Quiz reports include score distributions, pass and fail ratios, per-question difficulty, and completion versus abandonment rates.
